Current volunteers must have or have had an eye exam from their eye care provider within the past 12 months and have their provider complete the Vision Clearance Letter (VCL) and include documentation of the exam findings as well as complete the WakeMed Vehicle Operator Training course.

- Print Vision Clearance form and take to your eye exam or copy and send the link to your physician.
- Please include documentation of the exam findings with your form and email to Volunteer Services:

- To access the course, you will be sent an email with a link to the course for you to complete.
