Center for Community Health

Led by a team of highly experienced WakeMed clinicians, the Center for Community Health provides coordinated, holistic care to some of the Raleigh area’s most vulnerable residents while engaging community partners to study, learn and work together to reduce health inequities. It serves as a safety net for those without another safety net.

Bringing the best of WakeMed’s population health programs under one roof, the Center provides highly-coordinated care in collaboration with community partners for the success of our patients and a healthier community for all.

Our team-based care model includes:

  • Primary Care
  • Psychiatry
  • Trauma Therapy
  • Community Case Management
  • Homeless Engagement Assistance & Resource Team (H.E.A.R.T.)
  • Transitional Health
  • Community based interventions, education and outreach focused on collaborative efforts with community organizations and faith-based communities in high-risk census areas with demonstrated health inequities.

The programs are based on successful population health principles and trauma-informed care to meet the diverse and complex needs of the community’s highest risk patients suffering from homelessness, mental illness, and/or advanced chronic medical issues. Follow-up clinical care is provided in the office and through home visits, mobile health care services and in other community settings.

Individualized care plans are developed by clinicians with experience, training and expertise in caring for vulnerable populations with the goal of transitioning the patient to a long-term medical home with our partners for ongoing care, therefore, greatly reducing the need for more emergency visits and hospital readmissions.

Community Support

We are grateful to our WakeMed Foundation donors and the City of Raleigh for their generous donations and meaningful support that helped make the Center for Community Health a reality. To date:

  • $1 million has been donated by the WakeMed Foundation to fund start-up operations.
  • $1.8 million has been donated by the City of Raleigh through the ARPA Community Health Initiative program for capital improvements of the facility.

The future of health care is whole health. It’s focusing on prevention, wellness, health equity, and collaboration while improving access to care, addressing social drivers, and so much more. It’s moving beyond our hospital walls and bringing care directly to our community — where and when they need it most. Together, we can create a pathway to whole health and a healthier community.
