WakeMed Primary Care Referrals for Behavioral Health
WakeMed offers psychological services to adult and pediatric patients referred by our designated Primary Care physicians and advanced practice practitioners through the WakeMed Behavioral Health Collaborative Care Program. The Collaborative Care Model (CoCM) partners the primary care provider, the behavioral health care manager and a psychiatric consultant with the patient to provide coordinated and measurement driven monitoring and treatment for the following common concerns:
- Anxiety
- Depression
- Stress management
- Grief/Loss
- Difficulty sleeping
- Making changes that can improve overall health

Our psychology program is set up to support our specialty medical programs in Adult and Pediatric Weight Management and Pediatric Gastroenterology.
WakeMed Neuropsychology Referrals
Our Neuropsychology program is a sub-specialty of professional psychology that provides diagnostic assessment of cognitive, behavioral and emotional changes that may occur following injury or illness. Neuropsychological assessment is available with a physician referral for patients with a variety of medical illnesses or when cognitive change or impairment is evident.

Patients identified with substance use and/or mental health challenges while in one of our Emergency Departments may be referred immediately to the Behavioral Health Network for crisis stabilization following a risk assessment.
