We employ staff with home health backgrounds. Our staff members are bonded and insured. We do extensive background checks, including licensure and certification that is kept current and updated annually.

Because our health care professionals are focused on meeting the medical and rehabilitation needs of patients, they do not provide such services as house cleaning, food preparation and transportation.

At WakeMed Home Health, we employ expert clinicians who are true advocates for their patients.

Our patients receive care from people who are interested in promoting their comfort and safety. Most importantly, our staff serves as a link between the patient’s physician and family, and keeps the patient informed of his or her progress.

Continuity of Care in the Comfort of Home

WakeMed Home Health professionals file patients’ insurance claims and handle all the paperwork. If a patient meets all the necessary strict criteria, the federal Medicare and state Medicaid programs could cover his or her home health care. We have experienced staff to help the patient understand his or her insurance coverage.

The Personal Care Team 

When showering safely, making meals and remembering to take medications get harder to do, trust the WakeMed Personal Care professionals to help you stay as independent as possible in your home. We work with people in their house, apartment or residential living facility. Your WakeMed Personal Care team will include Certified Nursing Assistants (CNAs) who are highly trained and CPR certified. They are supervised by an experienced Home Health nurse. Before services begin, you and the nurse will meet to develop and mutually agree upon your plan of care. Your care plan can be altered at any time as your needs change. 

Fees 

$45 an hour (You may receive several services within the hour). 2-hour minimum per visit. There is an additional cost for medication management. WakeMed Personal Care services are billed in 15-minute increments.

Safety Assistance 

  • Certified medical aides can assist you with the following conditions/needs. 
  • History of seizures 
  • Risk of falling 
  • Oxygen therapy 
  • Bleeding precautions, fluid restrictions 
  • Need for universal precautions, infection control measures 
  • Help residents with cognitive decline make safe decisions and provide them with reminders 

Bathroom/Elimination Assistance 

  • Toileting 
  • Assist with bowel/time void program 
  • Catheter care 
  • Change adult diaper/brief 
  • Change condom catheter 
  • Cleanse perineal area 
  • Monitor/record intake and output 
  • Ostomy care

Activity & Mobility
