From March through April of 2025, Bob Short noticed a significant drop in his energy and stamina. Initially, he attributed this change to the natural effects of aging.

"My declining energy impacted me regarding stamina for my daily activities — working out; walking; doing yard work; golfing; and spending time with my wife, children and grandchildren. I even needed the occasional afternoon nap."

A Primary Care Warrior Who Cared

Not realizing these were symptoms of a deeper issue, Bob just happened to have an appointment on May 19 with his primary care physician John Whelan, MD, of WakeMed Primary Care – North Raleigh. It was a follow up to his annual visit on November 19, 2024. 

"Dr. Whelan asked me how I'd been feeling and if I had any concerns, so I casually mentioned the changes in my energy levels. Kudos to Dr. Whelan for reacting to my comment and ordering my first echocardiogram on June 3. I remember him stating, 'Let’s do one just in case.'"

Dr. WhelanDr. Whelan notes, "I ordered the echocardiogram because he had developed mild leg swelling and shortness of breath in addition to decreased energy. My patient's care is my top priority."

The results came back concerning, so Bob was referred for additional testing. 

On June 30, 2025, Andrew Sampson, MD, a WakeMed interventional cardiologist, referred Bob for a cardiac catheterization and performed it at the WakeMed Heart Center. He was diagnosed with moderate to severe aortic insufficiency with an ascending aortic root aneurysm, measuring 4.5 centimeters.

Dr. Sampson explains, "I referred Bob for cardiac catheterization because he had evidence of significant aortic insufficiency and an enlarged aortic root on echocardiogram that likely required surgical intervention. Coronary angiography was performed to evaluate for significant coronary artery disease prior to referring him for surgery to repair his aorta and aortic valve." 

About Aortic Insufficiency and Aneurysm

Aortic insufficiency, especially when paired with an ascending aortic root aneurysm, is a serious health issue. In this condition, the aortic valve doesn't close properly, allowing blood to flow back into the heart. This can cause the left ventricle to enlarge and weaken, which may eventually lead to heart failure. The aneurysm, characterized by the ballooning of the aortic root, often stems from the valve's malfunction, which can be due to congenital defects, age-related changes, or infections.

Surgery: The Choice. The Answer.

"I was shocked by the seriousness of my diagnosis. Sure, I'd experienced declining energy, and I had difficulty engaging in activities I'd enjoyed, but I never suspected that there was something seriously wrong with my heart."

Bob was offered treatment options, and he settled on aortic root repair and aortic valve replacement using bovine tissue. With this treatment protocol in mind, he and his wife began considering who would be the right surgeon.

"I'd never had heart issues before, but my wife had experienced aortic stenosis several years earlier and was treated at the WakeMed Heart Center. Bryon Boulton, MD, FACS, a WakeMed cardiovascular surgeon, operated on her, and he provided an excellent experience, so he was my top choice."

The couple consulted with Dr. Boulton. He answered their questions and scheduled Bob for surgery at the WakeMed Heart Center on July 18. It took three and half hours. 

"I felt surprisingly good after day two. I suspect that my minimal post-operative pain was due to Dr. Boulton’s use of a 'JACE' sternal closure."

JACE sternal plates were used during sternotomy to ensure that the bones were properly aligned, achieving accurate anatomical closure, while maintaining consistent dynamic compression throughout the procedure.

Meanwhile, Bob's hospitalization hit a curve when his recovery was interrupted by a brief bout of atrial fibrillation. It was resolved with cardioversion and medications, and Bob was discharged on July 22. 

"My three doctors were each wonderful. They were confident, reassuring and responsive to my questions — especially via My Chart."

"I embarked on my cardiac rehabilitation journey on September 4 and completed it on December 2 at the WakeMed Health Park in Raleigh. Over the course of 24 weeks, I participated in 48 bi-weekly sessions. The program emphasized the significance of a balanced diet, aerobic activities and strength-training exercises. 

"My primary exercise physiologist, Evan Bukowinski, led the sessions, supported by nurses Eleni, Carrie, Robyn and Teresa along with a dedicated team of therapists including, Patrick, Tara, Krista and Megan, among others who rotated through. The atmosphere was consistently uplifting, with the rehab team exuding enthusiasm for their work, which fostered a sense of comfort and optimism among all patients. They meticulously monitored our vital signs and progress, ensuring that each exercise was tailored to our individual capabilities and needs. Additionally, we received valuable coaching on dietary and lifestyle changes — essential for heart-healthy living."

Bukowinski adds, "Bob was a great patient and enjoyed walking and focused mostly on the treadmill and track for his cardiac modalities. He also enjoyed 20-30 minutes of resistance training with machines and free weights each session. For long term heart health, he will need to maintain his active lifestyle by staying consistent with his daily walking routine, attending his group aerobic classes two to three times a week, along with two to three days of resistance training a week."

A Hearty Return to a Happy Life

"Today, I feel rejuvenated and more like my former self. Having retired from a career in sales and federal contracts management with Fortune 500 technology companies, I now relish the time and health I have to enjoy life. My passions include traveling, playing golf, spending time at the beach, working out at the gym and engaging in activities with my grandchildren, ages 6,7,9 and 11."
