RALEIGH, N.C. (October 26, 2022) – WakeMed has opened the doors to the new Center for Community Health, Innovation, and Equity in Southeast Raleigh. Led by a team of highly experienced WakeMed clinicians, the Center for Community Health is designed to meet the diverse and complex needs of our most vulnerable residents suffering from homelessness, mental illness, and/or advanced chronic medical issues.

Located at 2600 New Bern Avenue, the Center for Community Health is on track to provide care to 400 to 500 people annually. The care team is based on an integrated care model including primary care, psychiatry, trauma-informed therapy, case management, homeless engagement assistance and resources, and transitional health as well as mobile and home health services along with community-based interventions, education, and outreach.

The Center serves as a safety net for sick patients without current primary care or community resources, bringing together many collaborative, community-based population health programs under one roof to enhance individual health and well-being, address social barriers, and put patients on a trajectory for long-term success.

“The Center for Community health is built on a foundation of community, collaboration, and innovation,” says Dr. Brian Klausner, executive medical director of WakeMed’s Center for Community Health, Innovation, and Equity. “We are working to meet the mental and physical health needs of our most vulnerable residents, while moving outside the walls of traditional health care facilities and into the community to better address long-standing health inequities.”

The renovated space also features trauma-informed design elements and aesthetics crafted to contribute to the patient’s sense of value, worth, and wholeness.

Funding for this innovative model of care and facility has been provided in part by the WakeMed Foundation and the City of Raleigh through the ARPA Federal Grant program.
