At WakeMed Physical Therapy in Clayton, we provide physical rehabilitation services to individuals who have pain or loss of physical function due to injury, illness or disease process. Treatment planning occurs through evaluating your goals, needs and potential for improvement within our program capabilities. Our programs are medically directed by a physiatrist and delivered by highly qualified professional staff who specialize in outpatient rehabilitation.

Services

During your initial visit, you will receive a comprehensive assessment and evaluation by each therapist prescribed for your care. You, your caregivers, if appropriate, and your therapist(s) identify treatment goals, discharge plans, treatment intensity, frequency and duration beginning with the assessment phase. Progress and goals are discussed with you during each visit. We will plan your discharge from the program when continued treatment is no longer medically necessary, treatment is no longer justified, and/or you are adequately prepared.

Family & Caregivers

Family and caregiver involvement in your care is encouraged. Your therapist can schedule extra time at your request to meet with your family or caregiver. Please request time in advance. Your therapist will also ask your permission before talking with your family about your care and progress.

For Parents

We do require a parent or guardian to stay on site during therapy. During the first visit, the therapist will meet with you and your child. During each subsequent therapy session, the therapist will review your child’s progress, and goals and provide activities to do at home. Please ask to attend treatment sessions if you have any concerns or questions.

If you feel our care has not been excellent, have concerns or have suggestions or comments on ways we can enhance or care and services, please talk with your therapist(s) or any member of our team. Our management team’s phone numbers are located on the back of this sheet. Other opportunities to share your feedback with us include:

  • Completion of a comment card (located in our lobby).

  • Participation in a random patient satisfaction telephone survey after discharge.

  • Call or meet with the facility manager, director or executive director to discuss any questions or concerns.
