Welcome! We'd like to take this opportunity to welcome you to our practice and to thank you for choosing WakeMed Heart & Vascular Physicians. We are constantly striving to make our patients' visits more efficient. We invite you to begin your new patient registration.

Prepare for your Office Visit 
In addition to completing the above forms, we will also require the following information for your fist visit:

  • Driver's License or Other Valid Government-Issued Photo Id
  • Insurance card and any co-payment due for visit
  • If applicable, any referral form
  • If applicable, any X-Rays, MRI Films, CT Scans, or EMG
  • If applicable, any pertinent medical history such as lab results

Please arrive 20 minutes early if you have not yet filled out these forms. If you have completed forms with you, you only need to check in with the reception desk 5 minutes before your scheduled appointment.

WakeMed MyChart - Online Patient Portal 

We offer an online patient portal called WakeMed MyChart to enable 24/7 access to a portion of your medical records for existing WakeMed Physician Practice patients. Patients of WakeMed Physician Practices can use WakeMed MyChart to:

  • Schedule or request appointments with WakeMed Physician Practices providers.
  • Save time by filling out health history information before your appointment.
  • View details from all outpatient appointments, including lab results, testing, diagnoses, medications, etc.
  • Communicate with your outpatient providers and request prescription refills by the click of a button.

To sign up, you will need a one-time activation code. If you are new to WakeMed Physician Practices, you will recieve your activation code after your first appointment. If you are a current WakeMed patient, you can request an activation code online.  Please note that the activation code will expire after two weeks. Remember, WakeMed MyChart is NOT to be used for urgent needs. For medical emergencies, dial 911.
