Stroke is a life changing event. In addition to physical issues, there are a range of emotions that impact both patients and their family. At WakeMed, we work with stroke patients at each phase of recovery to develop achievable goals that result in an optimal recovery.

We believe in a personalized team approach that includes both patients and families — from the moment a patient enters one of our emergency departments, to the moment they enter inpatient rehab or outpatient therapy.

Types of Stroke Rehabilitation

Depending on the severity of the stroke, patients will generally start with more intensive rehabilitation and gradually advance to outpatient and home-based therapy. Our WakeMed rehabilitation team skillfully and compassionately guides patients as they progress through our program.

Inpatient Rehabilitation Program 

A physician, hospital discharge planner/case manager, social worker or insurance company case manager can request an admissions evaluation. Once we’ve received a referral, an admissions representative will work with the patient’s physician, case manager, social workers and other care providers to determine the right level of rehabilitative care.

Outpatient Day Treatment Program

After a stroke, many patients benefit from our intensive, daylong therapy program. Our unique daylong outpatient program combines therapy and specialized reintegration support. The program helps patients regain their confidence and skills so they can return to work, school, driving and leisure activities.

This concentrated program has earned national accreditation by the Commission on Accreditation of Rehabilitation Facilities. Highlights include:

  • A team approach to care, including physical therapy, occupational therapy, speech-language pathology, case management and neuropsychology
  • Personalized community reintegration assistance to help patients and families cope and adjust, and to meet financial and medical needs
  • Supervised therapy by a physiatrist who specializes in rehabilitation
  • A specialized therapy team that provides wheelchair assessment, driving screenings, adaptive equipment and environmental and assistive technology

Outpatient Services Patients who no longer need hospitalization can continue therapy at one of WakeMed’s many outpatient rehabilitation centers located throughout the region. Each patient’s schedule is individualized and determined by evaluating therapists. Therapies include occupational therapy, physical therapy and speech therapy.

Home Health Services

Once a patient is no longer in need of hospitalization, he or she may be able to go home independently or with assistance from family or loved ones. WakeMed Home Health provides home-based therapy, including:

  • Intermittent skilled nursing care
  • Nurse aide services
  • Physical, occupational and speech therapy
  • Social work
  • Infusion services
  • Wound care
  • Nutrition therapy
  • Pain management

Our patient-customized approach and comprehensive case management service allows our patients to enjoy increased independence, safety and confidence. We take pride in providing care that is personal and professional, compassionate and convenient.

Our goal is to help patients regain their independence and the highest level of function in the comfort of their own home. Our staff serves as a link between the patient’s physician and family, and keeps the patient informed of his or her progress.

WakeMed Home Health is Medicare and Medicaid certified, state licensed and Joint Commission accredited, ensuring the highest quality of care. We are able to file patients’ insurance claims and handle all the paperwork.

Stroke Rehabilitation Facilities

Our supportive and healing environment is designed to provide excellent medical, emotional and spiritual care. Our facilities include:

  • Emergency Department: After a stroke, it’s critical to get quality medical care immediately. Our team is ready to provide a distinct plan of care that guides patients and families from the minute they enter our doors.
  • Primary Stroke Centers: WakeMed Raleigh Campus and Cary Hospital are both Joint Commission-certified Primary Stroke Centers. Learn what makes our Stroke Program  unique.
  • Neuro Care Unit: This acute hospital unit, located on our Raleigh Campus, is designed for individuals who are medically stable and can benefit from coordinated rehabilitative services, but who may need more time to recover before being ready to move to the next step. Each patient’s schedule accommodates his or her current level of functioning and provides occupational therapy, physical therapy and speech therapy.
  • Supportive Departments: Patients are supported through additional therapeutic care, including outpatient rehab, day treatment, fitness and wellness programs, wound care, home health and other supportive services.
  • WakeMed Rehabilitation Hospital: Our inpatient rehabilitation hospital, located on our Raleigh Campus, treats patients who need intensive rehabilitation. More than 30% of the patients we treat in our Rehabilitation Hospital are recovering from stroke. Our goal is to help patients become as independent as possible so they can return to daily life.

We strive to create a comfortable, homelike setting. Common areas are equipped with televisions and computers, and a private dining room allows families to host birthday parties, celebrations and other important special events. Patients and families have weekday access to our onsite HealthPark, which includes a fish pond, walking track, miniature golf course, ping pong table and multiple spaces to enjoy with family, friends and guests.

In our newly renovated gym space, patients undergo therapy that can include occupational therapy, physical therapy and speech therapy. Patients also have access to neuropsychological services and recreational therapy.

Our focus and attention to detail doesn’t stop at aesthetics. Our highly trained treatment team members deliver a level of care and compassion unmatched in our area. Nurses, therapists, aides, case managers, physicians and physician assistants are dedicated to assisting patients with getting back to life as quickly as possible.

Fitness After Stroke

WakeMed Healthworks - Raleigh Campus offers fitness equipment, group classes, personal training and specialty programs for patients recovering from stroke. Our fitness experts help patients design a fitness plan to rebuild body strength and muscle tone. With a structured plan that has specific goals, patients can return to a more normal lifestyle and discover activities and programs that are interesting and supportive of their recovery. We work one-on-one with each patient, helping to track progress, lending support and revising fitness goals as needed.

Clinical Case Management

Each patient has a stroke rehabilitation case manager who can provide support, information and referrals to hospital and community services. All of our rehab case managers are masters-prepared and licensed clinical social workers who work directly with patients and their family members. They can assist with:

  • Developing an individualized plan of care and treatment while a patient is in the rehab hospital
  • Understanding and navigating the financial impact and the health insurance coverage process
  • Setting individualized treatment goals and desired outcomes for the inpatient rehab stay
  • Preparing the patient and family for the return home
  • Discharge planning needs after hospitalization
