As of July 1, 2015, current diabetes patients requesting a Blood Glucose Log Review are required to do this through their MyChart account. Alternatively, you may schedule a visit with the provider or diabetes educator to review the blood sugars in person.

If your child is planning to obtain a driver's permit or license, the North Carolina Department of Motor Vehicles (NC DMV) currently requires individuals with any chronic health problem to complete a Medical Report Form (DL-78). This form can also be downloaded online at the NC DMV website, under the "Forms" section. This form requires signatures from both your child's diabetes doctor and primary care doctor.

There are several diabetes camps in North & South Carolina. These are great opportunities for children with diabetes (Type 1 and Type 2) to learn more about the disease and meet other children with diabetes.
A sliding scale is a "cheat sheet" to help you determine your child's insulin needs when they have a high blood sugar. You can download the more common Sliding Scales below.
If you child is sick, make sure you are checking blood sugars and ketones throughout the day, and encourage plenty of fluids. If you child has ketones present, our staff can provide you with assistance by calling our Nurse Line or Emergency Line at 919-350-7584 (available 24-hours a day, 7-days a week).
