Occupational Health


All WakeMed Health & Hospitals employees are required to provide proof of immunity to several communicable diseases. We suggest you start to gather your immunization records.  The only vaccination that is REQUIRED before you can begin employment is proof of COVID vaccination or an approved exemption.  All other records can be provided during the first week of employment. 

The enclosed Consent for the Release of Information form is provided to assist you in obtaining your immunization records from former providers, schools, employers, especially if you worked in a hospital setting.  If you would like to request your records be forwarded directly to WakeMed Occupational Health & Safety, please print the release form and send to the appropriate organization(s). 

In the event official records of immunizations or lab results are not provided, any applicable vaccines will be administered during the first month of employment. 

Listed below are the requirements for employment at WakeMed: 

Measles 2 Measles/Mumps/Rubella (MMR) vaccines or a blood test result indicating immunity to the Measles.
Mumps 2 MMR vaccines or a blood test result indicating immunity to the Mumps
Rubella 2 MMR vaccines or a blood test result indicating immunity to Rubella
Tdap 1 Adult Tetanus, Diphtheria, Pertussis vaccine in lifetime.
Tetanus Booster Td/Tdap Must have every 10 years
Chickenpox (Varicella) 2 Varicella Vaccines or a blood test result indicating immunity.
TB/PPD Test Must have 2 negative skin tests or negative QuantiFERON Gold (Q Gold) blood test in the previous 12 months.  If you have not been tested for TB in the past 1 months, you must visit Occupational Health to have a Q Gold blood test done within the first week of employment.  If history of a positive skin test or a positive Q Gold, must provide a copy of the date of positive result and a copy of most recent chest x-ray.  
Influenza Vaccine 1 influenza vaccine for the applicable influenza season or an approved request for exemption due to medical contraindications or a bona fide religious belief.
Hepatitis B Vaccine / Titer (Optional) 2 or 3 shot series or a positive hepatitis B surface antibody blood test.
