WakeMed Physician Practices – Primary Care – Brier Creek is pleased to offer a medical weight management program to help patients work toward a healthier life.

Unlike many traditional weight loss programs, medically-directed weight management programs are designed to foster close interaction between participants and a physician – offering a wealth of benefits including better overall health outcomes, healthy weight loss and a comprehensive approach you can’t get anywhere else.

Getting Started

Eligible participants will first attend a 30-minute individual intake appointment with the supervising physician. Insurance deductibles, co-insurance and copays apply as per usual for a primary care visit.

Is medical weight management for me?

Who’s eligible?

Any patient age 18 or older who has with a body mass index (BMI) greater than 30.

Why choose medical weight management?

Our medical weight management team is led by specially-trained primary care providers who understand that weight management is a complex process that is more successful when you take a person’s full health profile into consideration – including underlying medical conditions and health concerns, metabolic rate, and much more.

Participants enjoy a group setting where they benefit from collaboration, team support and ongoing education. One-on-one appointments are also available.

How does it work?

Eligible participants can choose between our group program (known as Shared Medical Appointments) or a one-on-one program as outlined below.

What can I expect from the group program?

Shared medical appointments offer a small, supportive setting of peers and a care team, education to help with ongoing weight management, and the convenience of medical management.

During your visit, the doctor can adjust your treatment plan, provide chronic disease management, watch for any complications and help you overcome obstacles as you face them. Depending on individual needs, you may be prescribed medications or prescription food supplements to help meet your weight loss goals.

Participants will first attend a thirty-minute intake appointment with the supervising physician. Once enrolled in the program, patients will attend a 90-minute shared medical appointment, also known as a group session, once a month for six months or as long as needed.

Each group session will include:

  • A 30-minute nurse-led lifestyle lesson focused on skills that will help you take control of your eating habits and improve your health. Topics will include nutrition, activity, meal planning, chronic disease management, behavioral aspects of weight issues and more.
  • A 60-minute physician-led medical appointment where each participant (6-8) will have a brief conversation with the physician to discuss their treatment plan and individual goals within the group setting. Participants get the benefit of hearing the answers to other participant’s questions, and the physician sometimes presents additional education to “go deeper” into the answers of
    these questions as compared to a traditional 15-minute individual physician appointment.

After six months, participants may be eligible to re-enroll in the program, if space is available.

What can I expect from the one-on-one program?

While we believe in our group model for many reasons, patients who don’t want to participate in the group visits can also elect to work with one of our providers personally for a one-on-one medical weight management program. After the initial 30-minute intake visit, appointments are 15 minutes and are scheduled as needed.

These appointments allow you to discuss your progress and goals, while also making changes to any medications or treatments you may be on related to your weight.

How much does it cost?

For each visit, participants will be charged insurance deductibles, co-insurance and/or co-pays as per usual for a primary care visit. Any recommended medications or prescription food supplements will be an additional charge.

Who can participate?

To be eligible for the program, you must have a body mass index (BMI) greater than 30 or meet other criteria. If you are interested in the program and think you may qualify, call 919-350-1000 to be pre-screened and to schedule a 30-minute intake appointment (a co-pay is required).

I’m interested in bariatric surgery. Is this program of any use to me?

Yes. In order for bariatric surgery to be covered by many insurance plans, you must have six to twelve months of medical weight management, which will be provided by this program.

The program can also be helpful to people who have recently had bariatric surgery and are looking for additional support following the surgery.
