Application Instructions & Checklist

  1. Apply online

    Apply for Residency
    Physician Assistant, Resident Surgery and Trauma

  2. Once you've applied online, please send an email to to let us know.  This will help us track all of your information.
  3. In order for your application to be considered complete, the following items are also needed: 
    • Physician Assistant program diploma (if applicable)
    • PANCE/PANRE certification (if applicable)
    • State licensures (if applicable)
    • Resume/CV (this may be uploaded when you apply for the position)
    • Transcripts (both undergrad & PA school)
    • Three (3) Reference Forms - These forms should be returned directly to our office using the mailing address provided.  Your references must include a PA Program faculty member and at least one supervising/precepting physician.
    • Essay - In one (1) page or less, please answer the following statement:  Please discuss your interest in pursuing additional training in a trauma, critical care, and general surgical physician assistant residency program.  Please also include any goals or ambitions.
