The year-long diabetes self-management program includes:
A one-hour initial assessment
One all-day education class
Complimentary follow-up sessions for ongoing support throughout the year with topics ranging from dental health and diabetes to holiday eating tips.
We provide education for individuals who have recently been diagnosed with diabetes, had a change in their treatment regimen, or are having difficulty maintaining glycemic control.
Basics of control: meal planning, exercise, blood glucose monitoring, medication and standards of care
Meal Planning: goals for healthy eating; food groups and portion sizes; the effect of carbohydrate on blood glucose levels; healthy eating guidelines for fat, cholesterol and sodium; how to read and interpret food labels
Benefits of exercise and how to exercise safely
Information about foot care
Importance of blood glucose monitoring; discuss blood glucose targets, testing schedule, record keeping, understanding the meaning of an A1c
Medication: how medication works to assist with diabetes control
Acute complications of diabetes and prevention strategies
Sick Day guidelines
Review of chronic complications from uncontrolled diabetes
All-Day Class - can attend all-day session or split class into two half-day sessions. An individual one-hour assessment with the nurse or dietitian is required prior to attending the class. Classes are available at the Raleigh Campus and Cary Hospital.
Follow-Up Sessions Class participants return at three, six and 12 months after class completion for an hour and a half followup to review progress toward self-identified behavior change goals and additional educational programming.

Reviewing goals and progress to date
Addressing questions and concerns
Solving problems and setting goals
Continuing education on a variety of diabetes topics
Cost The charge for instruction for the Diabetes Outpatient Self-Management Program is covered all, or in part, by most insurance companies, including Medicare and Medicaid. Follow-up sessions are free of charge.
