We also encourage patients to take a proactive role in their own health care and wellness activities.  We want to make sure your visit with us is as enjoyable as possible; therefore, we ask that you review the directions to our office, the information about our doctors, our office hours, and what documentation and information you should bring with you to your upcoming visit.

WHAT YOU NEED TO KNOW:
For your first visit, please review our Notice of Privacy Practices and bring the following items with you.

  • A copy of your insurance information
  • All of your medication bottles with you to each visit. This allows us to keep our records current and is especially important when physicians from other offices are providing medications for you.
  • Any old records including immunization records or documents from other physicians to your visit.

Subsequent Visits

  • Bring insurance information and all of your medication bottles with you.
  • Please note that if this is a follow-up appointment, the time is reserved specifically to monitor your medication use, and the schedule won't allow for discussion of other issues. You will need to schedule an additional visit for other problems.

Medication Refills

  • In general, your physician will prescribe enough medicine to last until your next visit. Once you start your last refill, it is time to contact our office. Please do not allow your medicine to run out. Plan on scheduling an appointment or calling at least 7 days before you run out.
  • Refills on controlled substances (i.e., narcotic pain medications) can be provided by your primary physician only, and must be obtained in person.
  • Antibiotic medications will not be prescribed by phone; your healthcare provider will need to examine you first.
  • Prescription refills will be handled during regular business hours only, ending at 3:30 pm. Requests received after this time will be handled the next business day.
  • The easiest and most efficient way to get prescription refills is to call your pharmacy. Call the pharmacy even if the bottle says no refills.
