Physicians desiring to make a referral to the WakeMed Children's Children's Endocrinology & Diabetes Center may click here for a referral form.  Please fax the completed form along with a growth chart and pertinent medical records to (919) 231-0314.

WakeMed Children's Endocrinology & Diabetes has a new Emergency On-Call process.  If you see a child in your office with new onset diabetes mellitus or are in need of an immediate endocrine consult,  call WakeMed's provider to provider service at 919-350-4P2P (350-4727) to be connected to the Children's Diabetes and Endocrinology Provider on call.  Our provider will decide whether your patient needs hospitalization or can be seen in our outpatient office.

If your call is not urgent, please call the Children's Endocrinology & Diabetes office at 919-350-7584 open Monday through Friday, 8 am to 4:30 pm.
