Current diabetes clinic patients can find blood glucose log sheets here. These can be printed and faxed to us at (919) 862-1202 after entering your child's blood glucose information on an as-needed basis.
Current diabetes clinic patients who are of driving age can find the Safe Driving Agreement here.
The WakeMed Children's Diabetes clinic provides care for children and adolescents with diabetes from birth to 18 years of age. We provide:

Patients new to the clinic click here for the diabetes clinic registration form. Please complete and bring with you or fax to (919) 862-1202 prior to your visit. If your child is not new to diabetes, please request medical records from your previous diabetes care provider.
