Our team consists of two board-certified pediatric endocrinologists, a certified diabetes educator, social worker, and additional clinical support staff who specialize in the management of children with diabetes and endocrine problems. Our clinic serves a diverse population of children, ranging from birth to 18 years of age, and is an American Diabetes Association (ADA) recognized program.
Patients new to the clinic, please download and complete the following forms prior to your initial clinic visit. You may fax these forms to (919) 231-0314 prior to your visit, or bring them with you. If your child’s endocrine or diabetes problem is not new, please request medical records from your previous endocrinology care provider.
