ENERGIZE! Program

The WakeMed Pediatric Diabetes Program is dedicated to helping kids ages 6 to 18 reduce their risk of developing type 2 diabetes.

We do this through "Energize!," a grant-supported program designed to teach young people how to build lifelong, healthy attitudes about food and fitness.

Energize! Goals:

  • Predict risk: Identity children ages 6 - 18 with type 2 diabetes, pre-diabetes or cardiometabolic risk factors
  • Provide an intensive, 12-week, family-based healthy lifestyle program for children at high risk
  • Prevent and treat type 2 diabetes through sustainable lifestyle change

FORMS:

  • Click here for more information on this 12-week session.
  • Your child's pediatrician will need to fill out and fax a referral form to get you started in the ENERGIZE! program. After we receive the referral, your child will need a fasting blood sugar test, as well as a fasting cholesterol screening.  This can be done in your physician's office or at any WakeMed lab.
  • Parents, if your child has already been referred to Energize! and you have scheduled an appointment with our staff, you can click here for the registration form and the parent and child physical activity and nutrition forms that you can complete proir to your appointment.

Program Highlights

The ENERGIZE! program begins with a physician referral and screening labwork.  Youth identified with elevated fasting glucose or metabolic syndrome (see referral form) qualify for the program, and their family attends an individual assessment visit with the educator to explain the program and evaluate their readiness to change.

ENERGIZE! is a 12-week, 3 night a week, community-based intensive education and lifestyle change program, including lessons in nutrition, physical fitness, and behavior change education.

The group program takes place in a local community center (YMCA, Parks and Rec, YWCA) convenient to the child's home, and is taught collaboratively by WakeMed clinicians and fitness partners at each site.

Families are integrated into the program through nutrition classes and fitness offerings.  WakeMed provides program oversight and the nutrition and behavior change curriculum and training for all of the fitness partners.  Fitness partners provide the fitness activities and direct the fitness portion of the intervention using resources unique to their facilities and expertise, along with fitness curriculum provided by WakeMed.

Follow-up begins upon completion of the program, and visits are scheduled every six months for two years to obtain BMI, blood pressure, fasting glucose and lipids, and assess progress with nutrition, physical activity, and behavior change goals.
