Discharge: Care Resources
At WakeMed, we take pride in providing exceptional care. This responsibility does not end after you have been discharged from the hospital. Even after discharge, we want to provide you with access to the best care for your particular situation to ensure a smooth and steady recovery.
Physician Follow-Up - Often before you leave the hospital, your nurse will work with you to make follow-up appointments with your physicians. It is very important that you visit your doctor at the scheduled time. If you are unable to make the scheduled appointment time, please contact the physician office directly to schedule a new appointment. Your safe recovery depends of follow-up care.
Home Health Referral - WakeMed Home Health gives you connections to qualified and experienced staff who know how to adapt to a person's home environment, connect to major medical centers with immediate access to experts and specialists, and connect to a state-of-the-art rehabilitation center. WakeMed Home Health offers skilled nursing, home health aides, physical therapy, speech-language pathology, occupational therapy, clinical social work, infusion services, pain management, wound care and much more.
Transitional Care - WakeMed has partnered with Paired Health to successfully and rapidly help patients transition from hospital to home. Paired Health is unique in that a primary care physician, physician assistant or nurse practitioner makes house calls. Paired Health providers serve as a transitional primary care provider for patients when the primary care provider is not easily accessible due to home support, transportation or physician appointment availability. Your case manager will discuss with you whether or not transitional care is recommended for you, and will make the appropriate referral to Paired Health.
