For Referring Physicians
Physicians desiring to make a referral to the WakeMed Pediatric Diabetes and Endocrinology Center may click here for a referral form. Please fax the completed form along with a growth chart and pertinent medical records to (919) 231-0314.
If you see a child in your office with new onset diabetes mellitus or are in need of immediate endocrine consultation, please call our 24-hour pager at (919) 393-0222. Our on-call clinician and pediatric endocrinologist will decide whether your patient needs hospitalization for stabilization or they can be managed by us on a same day basis as an outpatient. Physicians in need of endocrine phone consultation will be put in direct contact with our pediatric endocrinologist.
