New Warfarin Order Sets Available

WARFARIN is the most common cause of adverse drug events - please help us improve safety by using the new Warfarin Order set.  Pharmacists will assist with the implementation of this transition by placing these orders on your warfarin patient's charts.  In addition, this order set will be available on the nursing units.

H1N1 Update

CDC has identified oseltamivir resistance in pandemic influenza A (H1N1) isolates from two epidemiologically linked patients in NC.

LV function assessment documented, planned or reason documented for no assessment. Many ESRD patients or those with "fluid overload" are often coded as heart failure - please document known EF on these patients.
